RACE CAR SUBMISSION FORM

Before your car can be entered in CSRG events
it needs to be accepted by the Board of Directors.
Use this form to submit your car.

PRE-TECHNICAL INSPECTION WORKSHEET

MEDICAL FORM

You need a current, valid CSRG or other Vintage Motorsports Coucil affiliated club Medical Card to participate in a CSRG event.
A current, valid SCCA or FIA Medical Card is also accepted.

To get a CSRG Medical Card, you must be a current CSRG Member.
Your doctor must perform a physical examination and complete,
sign, date and stamp a CSRG Medical Form,
making sure to initial pages 1, 2 and 4 in the space provided.

You can get a CSRG Medical Form by downloading it in PDF format
and printing all four pages; or you can get a pre-printed form by calling
1-888-268-7126, pressing 3 on your keypad, and leaving
a message with your name and address.

An original, ink-signed Medical Form with all four pages
stapled together is required for our insurance files.
We cannot accept photo copies, scanned or faxed copies
of either your or the Examining Physician’s signature.

Please note that SCCA Medical Forms are NOT accepted by any Vintage Motorsports Council affiliated club for the issuance of a Medical Card.
