Below are all the CSRG forms you will need,
including a short introduction to each form to help to decide
which form to download and to print.
(All forms available online, except the Medical form.

Before your car can be entered in CSRG events
it needs to be accepted by the Board of Directors.
Use this form to submit your car.

You need a current, valid CSRG or VMC affiliated club Medical Card to participate in a CSRG event. To get a CSRG Medical Card, your doctor must perform a physical examination and complete, sign, date and stamp a CSRG Medical Form.
An original, "wet-signed" copy of the Medical Form is required for our insurance files. We cannot accept Xerox copies or faxed copies of the form.
Please note that SCCA forms are NOT accepted by any Vintage Motorsports Council affiliated club for the issuance of a medical card.

for each event..
Be sure to always download the REQUEST FOR ENTRY form for the event you wish to enter. This form includes information on the driver, car, entry deadlines and fees.
